REQUIRED FORM - PLEASE SUBMIT TO INITIATE GROUP REGISTRATION PROCESS

Event: SPRING UNITREAT April 4-6, 2025
Location: Lutherdale, Elkhorn, WI 

Registration Deadlines: Feb 28th for early bird, Mar 7th for regular.

YFM Director or Designee in charge of registration for your chapter: Please have already identified who is attending before filling out this form.
Please List all Uniteens (6th-8th graders) and Adults below who are planning to attend, plus any high schoolers who are applying be on YOU staff. Unitreats are soley for Uniteens, with 3-4 YOUers on staff to help the Regies lead spirit groups and funshops.

Once submitted, substitutions and additions must be approved by your Church Minister/Spiritual Leader and Cassidy Meeks, Youth and Family Ministry Coordinator.

The person listed below will be asked to attest to the following:
1. their understanding of the Heart Agreements for the youth and Leaders
2. their support for each person attending as a representative in good standing with the church/center. 
3. all adult participants have a National Criminal Background Check on file with this church and
4. That all youth participants listed are active members of the Youth Program, meet the regional/local eligibility requirements and are spiritually mature and responsible individuals prepared to attend such an event. 

Adults attending: The Region recommends 2 adults travel with your group. Co-ed groups asked to provide co-ed leadership. Drivers must be 25 years of age or older. A background check for each adult must be on file with your church/spiritual center.

Please submit ONE payment for your entire chapter either:

1. By Check a. Make it out to: Unity Worldwide Ministries Great Lakes Region and list "Spring Unitreat" in the Memo

b. Mail checks to: GLUR, N1886 Municipal Drive, #162, Greenville, WI 54942

2. Online a. go to greatlakesunity.com and click donate. b. Select ‘youth event registration’ from dropdown and input credit card information. You do not have to pay extra to cover credit card fees (though you're welcome to do so). 

Regardless of how you pay the Region, please first collect payment from families and submit one payment.

Your group’s registration fee is expected by the date registration closes, so please plan accordingly. If your payment will not be in by that date, please contact Cassidy as soon as possible.

The following is a list of opportunities which chapters can volunteer for at the event. Please discuss with your group and select your 3 top choices. You will receive more detailed instructions with your confirmation packet.
