To help ensure a more wonderful experience for all,

I agree to hold a meeting with all participants and their parents/guardians before retreat to go over the heart agreements in detail, or otherwise ensure that all participants are clear about the expectations of this event.

All participants MUST have a Ministerial or Spiritual Leader endorsement to attend this event. Your Minister/Spiritual Leader will be asked to attest to the following:
1. their understanding of the Heart Agreements for the youth and Leaders
2. their support for each person attending as a representative in good standing with the church/center.
3. that all adult participants have a National Criminal Background Check on file with this church and
4. that all youth participants listed are active members of the Youth Program, meet the regional/local eligibility requirements and are spiritually mature and responsible individuals prepared to attend such an event.

We are thriled that scholarship funds are available from the Great Lakes Region to support youth who otherwise could not attend, and/or to help with transportation costs. Scholarship applications are due 8 weeks prior to the event. Learn more and apply

Chapter Service Opportunities

The following is a list of opportunities which chapters can volunteer for at the event. Please discuss with your group and select your 3 top choices. You will receive more detailed instructions with your confirmation packet.

Service Opportunities

Snacks – bring a snack for Saturday evening celebration. Snacks (other than fresh veggies) should be packaged, and must have a label that indicates all ingredients Strike Crew - help with taking down equipment and loading regional vehicle at the event's conclusion (will need to stay at least ½ hour past closing) Gear Angels – Bring extra bedding and towels for those who might forget Provide A/V Support Craft Table - bring craft items such as paper, ribbons, stickers, etc. and help to maintain its orderliness throughout the weekend. PROCESS: You will need to have already determined who from your Chapter is attending BEFORE filling out this form.

STEPS:
1. List ALL Participants - both Youth of Unity (6th-12 grades) and Adults - who are planning to attend.

2. Calculate Payment ($250/person) and Submit via Credit Card below, or Select Check Payment. If paying by check make out to "Unity Great Lakes Region" and mail to P.O. Box 162, Greenville, WI 54942.
Be sure to collect all payments from participants beforehand so your payment can be made by the Oct 4th deadline.

3. Send your participants the individual registration link, which is also due by Oct 4th. You may wish to bump up the registration deadline to your participants in order to meet the Region's deadline.
Participant substitutions and/or additions must be approved by your Church Minister/Spiritual Leader and the Regional Event Coordinator.
